Pediatric Drug Interactions with Cyproheptadine

• Avoid combination with MAOIs—risk of severe hypertensive reactions and potentiated anticholinergic effects.
• Monitor when combined with CNS depressants (opioids, benzodiazepines, alcohol) for additive sedation and respiratory depression.
• Limit coadministration with other anticholinergic agents (TCAs, antipsychotics) to prevent urinary retention or ileus.
• Cyproheptadine may blunt response to serotonergic migraine therapies (triptans) or serotonergic antidepressants; monitor efficacy.
