Documented anaphylaxis, angioedema, or severe cutaneous adverse reaction to dicloxacillin or any penicillin

History of immediate IgE-mediated reaction to other beta-lactams—consider graded challenge or alternate agent
Previous cholestatic hepatitis or liver dysfunction with antistaphylococcal penicillins
Severe renal impairment—no formal adjustment but monitor closely when high doses used
Asthma or atopy—screen carefully for prior allergy symptoms
Prolonged therapy in hepatic or renal disease—monitor liver enzymes, renal function, and CBC
Generally considered compatible; if breastfeeding, monitor infant for diarrhea, candidiasis, or rash.
