Esomeprazole is the S-enantiomer of omeprazole that concentrates in gastric parietal cell canaliculi, where it is protonated to a reactive sulfenamide that covalently binds the H+/K+-ATPase proton pump. Inactivation of the pump blocks the final pathway of acid secretion until new pumps are synthesized.
