Absolute: sulfonamide hypersensitivity, anuria. Relative: severe renal/hepatic impairment, electrolyte imbalance, gout, and pregnancy when alternatives available.

Contraindications

Absolute Contraindications (Never Use)

Hypersensitivity to hydrochlorothiazide or sulfonamide-derived drugs

Includes anaphylaxis, Stevens-Johnson syndrome, or angioedema with thiazides.

Anuria

Ineffective when kidneys cannot produce urine; avoid use.

Relative Contraindications (Use With Caution)

Severe renal impairment (CrCl <30 mL/min/1.73 mยฒ)

Diminished efficacy and increased risk of azotemia; consider loop diuretics instead.

Electrolyte depletion (hyponatremia, hypokalemia)

Correct imbalances before initiating to prevent arrhythmias or neurologic complications.

History of gout or hyperuricemia

Thiazides elevate uric acid; monitor levels and consider prophylaxis if recurrent gout occurs.

Diabetes mellitus or impaired glucose tolerance

Monitor fasting glucose as thiazides may worsen glycemic control.
