Modified PEWS is a pediatric early warning score that combines age-based vitals with exam findings and risk factors to identify children at risk of clinical deterioration.
This tool uses the YKHC age bands from 0-2 months through over 15 years, with separate score cards for each band.
Oxygen scoring uses the measured SpO2 and assigns 2 points if the child is on supplemental oxygen, regardless of saturation.
PMH points are added only when those risk factors are present. If none apply, leave all PMH boxes unchecked.
Document total score, domain drivers, intervention timing, and trend direction. This is more reliable for handoff and escalation than a total score alone.
