Iron supplements and fortified multivitamins
ModerateFerrous sulfate, Ferrous fumarate, Polysaccharide iron complex, Prenatal vitaminsMechanism: Bivalent cations chelate cefdinir in the gut lumen, cutting bioavailability by up to 80% and causing rust-colored, non-bloody stools as the iron–drug complex is excreted.
Management: Administer cefdinir at least 2 hours before or after iron-containing products. If separation is impossible, consider extending therapy 1–2 extra days to reach equivalent exposure.
Monitoring: Assess clinical response (fever curve, symptom relief) after 48–72 hours; lack of improvement may signal poor absorption.
Parent tips: Warn caregivers that stools can turn brick-red when iron and cefdinir mix—this is benign unless accompanied by pain or black, tarry stool.
Pediatric considerations: Premature infants and toddlers on chronic iron therapy are at greatest risk because they often receive dosing via bottles around the same time as antibiotics.
Laboratory monitoring: None typically required
